Capital City Energy Group, either directly or through one of its affiliates or subsidiaries (collectively, “Capital City”), from time to time, may request personally identifiable information from individuals with whom it conducts business, including, without limitation, such individual’s social security number and/or tax identification number (“Personal Information”). Capital City is committed to implementing commercially reasonable measures to maintain the confidentiality and security of Personal Information as set forth in this Privacy Policy. By providing your Personal Information either directly or through your representative to Capital City you acknowledge and agree with the terms of this Privacy Policy and consent to Capital City’s use and disclosure of your Personal Information, subject to the limitations stated in this Privacy Policy

Capital City obtains most Personal Information directly from you or your representative and may also collect public and nonpublic Personal Information about you from any of the following sources:

• Division orders, leases, inquiries, public records and other documents submitted by an individual or their representative or gathered by Capital City in the course of managing ownership interests and revenue distributions associated with a Capital City Energy Fund.
• Other interactions with Capital City

The collection of Personal Information is necessary for Capital City to fulfill certain legal and regulatory requirements including, without limitation, its obligation to report payments made to individuals to the Internal Revenue Service and other taxing authorities. You acknowledge and agree that Capital City will use and provide such Personal Information to governmental agencies, including the Internal Revenue Service, when filing Form 1099’s for payments such as revenue distributions.

Capital City has implemented physical, electronic and procedural safeguards in an effort to prevent the loss, misuse, or alteration of Personal Information in our custody and direct control. It is Capital City’s policy applied to our staff, to restrict access to Personal Information to those individuals who require access in order to manage ownership records and payments made by Capital City to individuals or to otherwise conduct business with its clients. Our staff is required to access such Personal Information only when there is an appropriate reason to do so, to protect the confidentiality of Personal Information, and to comply with Capital City’s Privacy Policy. Violations of our Privacy Policy are dealt with seriously.

Capital City may share Personal Information that we collect with the following entities:

• Affiliates or subsidiary companies to Capital City Energy Group

Without your consent, Capital City will not make disclosures of Personal Information to other companies who may want to sell their products and services (for example, Capital City does not sell customer lists and will not sell your name to a catalogue company or telemarketer.

You do not have to contact us to benefit from Capital City’s Privacy Policy. This Privacy Policy applies to individuals providing Personal Information to Capital City in order to receive revenue distributions or other payments from Capital City associated with ownership interests in a Capital City Energy Fund or such an individual that may have in oil and gas leases or properties or to receive payments for services provided on behalf of, or to, Capital City.

Capital City reserves the right from time to time to change this Privacy Policy. Capital City will endeavor to post a notice on our web site’s home page in the event of a material change. The examples contained in this Privacy Policy are illustrations and they are not intended to be exclusive. If this Privacy Policy conflicts with a contract with an individual providing Personal Information or provision of law, such contract or law shall supersede and control over this policy.
